About this apprenticeship

Coordinating and providing care and reporting that is safe, timely, effective, efficient, equitable and client-centred
Handling case assignments and referrals, drafting service plans / agreements, review case progress, co-ordinate with other care providers and determine case closure
Helping clients and employees achieve wellness, autonomy and client satisfaction
Support and improve multiple care aspects (case co-ordination, client resources, diary / resource management, information governance, etc)
Help the business communicate effectively with clients, patients, clinicians and the healthcare marketplace
Develop effective working relations and cooperate with medical teams to deliver continuous improvement in business performance and patient care
Interact with patients and clients to keep track of their progress, service provision and to ensure satisfaction

Training

This is a full-time, office based role in Guildford, Surrey, at Occupational Health Assessment Ltd, Surrey Technology Centre, 40 Occam Road, Guildford. GU2 7YG
One day a week is provided for off-the-job-learning, as well as regular group workshops with Surrey Adult Learning in Woking
